For HIGH SCHOOL and ELEMENTARY

• Must be a resident of Cagayan de Oro City
• Must not be a 4P ‘s beneficiary
• The school where she/he enrolled must be within the jurisdiction of Cagayan de Oro City
• Must be an enrollee of a public school
• Family should have at least two (2) students for the particular school year BUT only one (1) child per family can avail the assistance
• Amount of Assistance;

Note:  Assistance intended for school materials              

REQUIREMENTS:

1.   Original copy of Certificate of Enrolment from the class adviser or School Principal

2.   Original Copy of Barangay Indigency Certificate

3.   Grade of the previous school year

                        UPON AVAILMENT:   Bring (1) any valid government issued ID (not expired) or voters certificate or Barangay Clearance in the name of the claimant

For SENIOR HIGH and COLLEGE

• Must be a College student or Senior High School Student Enrolled in Private or Public School with outstanding balance;
• Must be a resident of Cagayan de Oro City
• The school where he/she enrolls must be within the jurisdiction of Cagayan de Oro City
• Must NOT be a Beneficiary of the following:
  • Special program for Employment of students (SPES)
  • City Scholar or the 5,000.00 educational assistance of the City Scholars Office(CSO)
  • CHED Scholarship

• > Application will only be open during enrolment period

REQUIREMENTS:

          1. Photocopy of Certification of Registration (COR) from the school with Statement of Account;

2. Photocopy of updated School ID

3. Official Good Moral Certificate issued by the School Registrar

4. Grades signed by the School Registrar

5. Barangay Indigency Certificate;

6. Photocopy of 4Ps ID (if the family is a beneficiary); 

7. Certificate of No-Business from the City Treasurer’s Office (Parents name)

8. Social Case Study Report (If the requirements are completed at CSWD)
